FDA issued an early alert on October 15 covering the Commander M delivery system, the catheter that carries the Edwards SAPIEN M3 valve across the septum and into the mitral position. Edwards has received three reports of the balloon bursting during inflation. One patient died and another had a serious injury.
Edwards told customers to stop using the affected lots immediately. A burst balloon can release fragments into the circulation, or leave the device impossible to withdraw through the guide sheath, which turns the case into open surgery. These patients were routed to a transcatheter valve because they were poor surgical candidates, so the fallback carries its own risk of death.
FDA approved SAPIEN M3 on December 23, 2025, the first transseptal transcatheter therapy for mitral regurgitation to clear the agency. The alert reflects reports through October 5, which puts a delivery system failure inside the first ten months of commercial use. Early alerts exist to move a signal like this before a recall gets classified.
