FDA sent Olympus a warning letter dated October 7 over quality management system violations found at six facilities. The agency inspected those sites late in 2025. They include endoscope production and management locations in Japan, Olympus Winter & Ibe in Hamburg, and Gyrus ACMI in Minnesota, which Olympus bought in 2008.

The letter addresses how Olympus answered the Form 483 observations from those inspections. Olympus says corrective and preventive actions are already in place and that it will file a response this month. The company says the letter does not restrict manufacturing, sales or distribution, and does not require recalls or new clearances. It had not appeared in the FDA public database as of October 13.

A warning letter at this stage says FDA judged the 483 responses inadequate, not that it found something new. The agency reads what a company sends back, and a thin reply is what turns observations into a public letter. Olympus has collected several endoscope-related warning letters in recent years, plus 2025 import alerts covering certain Japan-made laparoscopes and automated endoscope reprocessors.
